Course Description
As the Hank Williams Jr song says, what we need is “a little less talk and a lot more action.” In the Science of High Reliability we explored the fundamentals of human performance in complex systems. This course builds on that foundation through a series of practical applications of reliability concepts. Participants will work through actual case studies in the application human reliability methods and receive the knowledge & skills necessary to apply these methods to their care setting. This program will also show how several healthcare systems are now using reliability bundles (human error prevention behaviors) to improve quality outcomes and have reduced serious events by 48-91% in two years. You will learn to:
- Choose appropriate reliability behaviors using aggregating analysis of harm events
- Implement both a leader and a physician reliability bundle
- Build collaborative interactive teams (CIT’s)
- Simplify protocols by using human factors for written guidance documents
- Error proof an environment of care
- Increase the reliability of checks & verification
- Use cause analysis methodology in peer review
- Quantify the reliability of a time-out using probabilistic safety assessment (PSA)
- Use reverse tracer methodology to measure process reliability
